Job summary

Reema Health seeks a Community Health Navigator to build relationships with members and guide them toward greater engagement in their own care.

You will work with a growing team to address social barriers, schedule visits, document outcomes in the Reema App, and collaborate with community resources. Roles evolve quickly as the startup grows, offering meaningful, field-based engagement in Covington area.

Qualifications

• 18 years of age or older
• Available to work Monday–Friday during core business hours
• Valid driver’s license, insurance, and reliable transportation
• Contingent upon passing pre-employment drug screening and background check
• Comply with HIPAA and patient confidentiality requirements
• Use technology-based tools daily
• Lift and carry up to 25 lbs occasionally

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model
• Build and manage a caseload of 80-120 members
• Assess member needs, including social determinants of health, to build care plans
• Establish trusting relationships and follow through in a timely manner
• Help members understand the importance of health activities and overcome hesitations
• Coordinate referrals and appointment scheduling and attendance
• Organize workload to hit monthly goals and daily metrics
• Engage members via phone, text, email, and in-person visits
• Connect members to community resources and transport as needed
• Maintain professional boundaries and document all work in the Reema App
• Comply with confidentiality standards and HIPAA
• Represent Reema Well with independence in a fast-paced startup
