Job summary

Reema Health, Inc. seeks a Community Health Navigator to build genuine relationships with members, guiding them toward greater engagement in their own care using Reema's platform.

This remote, startup-style role offers local travel (25–50%), a caseload of 80–120 members, and collaboration with a multidisciplinary team to meet monthly goals while maintaining dignity and confidentiality. Strong communication and empathy are essential.

Qualifications

• 18 years of age or older.
• Field-based role requiring valid driver’s license, insurance and reliable transportation.
• Pre-employment drug screening and background check required.
• HIPAA compliance and patient confidentiality responsibilities.

Responsibilities

• Conduct phone and in-person outreach to enroll members.
• Manage a caseload of 80–120 members and provide coverage for others as needed.
• Assess member needs to build a member-centered care plan.
• Coordinate referrals and schedule appointments.
• Document all member work in the Reema App and keep care plans current.
• Maintain HIPAA confidentiality and protect information.
