Job summary

Guidehealth is seeking a Remote Clinical Pharmacist to advance HEDIS medication adherence initiatives and quality measures. The role requires a PharmD and active licensure, with MTM experience and strong data analysis skills.

Qualifications

• PharmD degree from an accredited school of pharmacy.
• Active pharmacist license in TX, PA, AL or TN.
• Minimum 2 years of HEDIS-related medication adherence or population health work.
• Strong knowledge of HEDIS measures related to adherence.
• Proficiency with EHR systems and data analysis tools.
• Excellent communication, problem-solving and organizational skills.

Responsibilities

• Lead efforts to improve HEDIS medication adherence and related quality measures.
• Analyze adherence data, identify gaps, and implement targeted interventions.
• Collaborate with providers and care teams to promote adherence to therapies.
• Conduct MTM sessions to address barriers and provide patient education.
• Identify and resolve adherence issues including cost and dosing schedules.
• Use motivational interviewing to encourage adherence.
