Job summary

Guidehealth, a data-powered healthcare company, is seeking a remote Clinical Pharmacist to drive HEDIS medication adherence initiatives and improve quality measures. The ideal candidate holds a PharmD, active pharmacy license, and at least 2 years in HEDIS-related adherence work.

You will analyze data, perform MTM sessions, and collaborate with care teams to optimize outcomes. This full-time, remote role offers competitive compensation and comprehensive benefits.

Qualifications

• PharmD degree from an accredited school of pharmacy.
• Active pharmacist license in the United States (willing to obtain TX, TN, AL and PA).
• Minimum of 2 years of experience in HEDIS-related medication adherence work, quality improvement, or population health.
• Strong knowledge of HEDIS measures, particularly those related to medication adherence.
• Proficiency in using EHR systems and data analysis tools.
• Excellent communication, problem-solving, and organizational skills.
• Ability to work independently as well as in a team-oriented environment.

Responsibilities

• Lead efforts to improve HEDIS medication adherence and other pharmacy quality measures.
• Analyze medication adherence data, identify gaps in care, and implement targeted interventions.
• Collaborate with healthcare providers, care managers, and other stakeholders to promote adherence to prescribed therapies.
• Conduct MTM sessions to address barriers to adherence and educate patients.
• Work directly with patients to resolve adherence issues including cost and dosage schedules.
• Utilize motivational interviewing techniques to encourage adherence.
