Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telehealth medication counseling, MTM, and adherence support from a home-based setting. You will engage with patients via phone or video, monitor outcomes, and collaborate with care teams to optimize therapy.

The role emphasizes strong patient communication, empathy, and virtual rapport-building, with a fully remote schedule and opportunities for professional growth.

Qualifications

Graduated PharmD or Bachelor of Pharmacy.
Active U.S. pharmacist license; multiple state licenses preferred but not required.
Ambulatory care, MTM, or clinical experience preferred.
Excellent communication and ability to build rapport virtually.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document all interventions and communications according to organizational standards.
Support population-health and disease-management programs.
