Job summary

Guidehealth, a remote-first healthcare company, seeks a Clinical Pharmacist to lead HEDIS medication adherence initiatives and MTM sessions. The role focuses on improving adherence for antihypertensive, cholesterol, and diabetes therapies, leveraging EHR data and collaboration with care teams to drive quality outcomes.

The candidate will monitor metrics, train staff, and serve as a SME in medication adherence, supporting population health and value-based care programs.

Qualifications

• PharmD required and licensure in at least one state (TX/PA/AL/TN).
• Minimum 2 years in HEDIS-related medication adherence or population health.
• Strong knowledge of HEDIS measures related to adherence.
• Proficient with EHRs and data analysis tools.

Responsibilities

• Lead efforts to improve HEDIS medication adherence and other pharmacy quality measures.
• Analyze adherence data, identify gaps, and implement targeted interventions.
• Collaborate with providers and care managers to promote adherence to therapies.
• Conduct MTM sessions to address barriers and educate patients.
• Work with patients to resolve adherence issues including cost and dosing.
• Use motivational interviewing to encourage treatment adherence.
• Monitor HEDIS performance metrics and generate reports.
• Provide updates to leadership and act as SME on adherence.
