Job summary

Rangam is seeking a Remote Clinical Pharmacist to drive Star/HEDIS initiatives within the pharmacy department across health systems and providers. You will coordinate with Magellan and health plans to improve Medicare Part D Star measures, including adherence and MTM activities, while managing outreach and evaluation of interventions.

Responsibilities include leading clinical solutions, monitoring program outcomes, and reporting to stakeholders.

Qualifications

• Experience in Health plan or Medicare clinical customer service or Retail management
• CMS Star/HEDIS measure quality improvement metrics
• Pharmacist license in New Jersey
• 2-5 years experience in related roles
• Experience coordinating between pharmacy, health systems and vendors
• Ability to analyze and report on interventions and outcomes

Responsibilities

• Manage outreach for Star/HEDIS clinical programs and drive 5-Star results
• Oversee daily clinical activities to meet Star/HEDIS goals across health systems
• Serve as clinical liaison between pharmacy department and health systems
• Develop and monitor clinical programs, pathways of care and drug use policies
• Track and report patient interventions and disease management findings
• Develop metrics with client data team to monitor quality, care and costs
• Conduct outreach/interventions and Comprehensive Medication Reviews as needed
• Communicate program design and execution across departments
