Job summary

Guidehealth is seeking a Clinical Pharmacist with a PharmD to support HEDIS medication adherence initiatives, ensure compliance with quality measures, and drive better health outcomes in a remote-first environment.

You will lead HEDIS quality improvement efforts, conduct MTM sessions with patients, analyze data to identify gaps, and train staff on best practices, while collaborating with care teams to optimize adherence.

Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
• Active pharmacist license in TX, PA, AL, TN.
• Minimum of 2 years of experience in HEDIS-related medication adherence work, quality improvement, or population health.
• Strong knowledge of HEDIS measures related to medication adherence.
• Proficiency in using EHR systems and data analysis tools.
• Excellent communication, problem-solving, and organizational skills.
• Ability to work independently and in a team-oriented environment.
• Experience with motivational interviewing or other patient engagement techniques is a plus.

Responsibilities

• HEDIS Quality Measures: Lead efforts to improve HEDIS medication adherence and other pharmacy quality measures, including adherence to antihypertensive, cholesterol-lowering, and diabetes medications; analyze data to identify gaps in care and implement targeted interventions; collaborate with healthcare providers and care managers to promote adherence.
• Patient Engagement: Conduct medication therapy management (MTM) sessions, address barriers to adherence, provide patient education, and ensure understanding of regimens; identify and resolve issues related to adherence, including cost, side effects, and dosing; use motivational interviewing techniques to encourage adherence.
• Data Analysis & Reporting: Monitor HEDIS performance metrics, generate reports to assess intervention effectiveness, use EHR and pharmacy data to identify patients at risk of non-adherence, and provide progress updates to leadership with improvement recommendations.
• Collaboration & Training: Serve as a medication adherence subject matter expert; provide training to pharmacists and clinical staff on best practices; participate in cross-functional teams on quality improvement and population health initiatives.
