Job summary

Pharmacy Careers seeks a Remote Patient Care Pharmacist to deliver medication counseling, adherence support, and clinical monitoring from home via telehealth. Flexible scheduling and direct patient care are core aspects of the role.

You will perform MTM, reconciliations, and collaborate with care teams to optimize treatment plans. A PharmD or B.Pharm, active US license, and strong virtual communication are required; ambulatory care experience is preferred.

Qualifications

• PharmD or B.Pharm degree required.
• Active US pharmacist license; multi-state licenses preferred.
• Experience in ambulatory care, MTM, or clinical settings preferred.
• Strong communication, empathy, and virtual rapport-building skills.

Responsibilities

• Deliver telephonic or video-based medication counseling to patients.
• Monitor therapy outcomes and adherence; propose optimizations.
• Perform medication reconciliations for high-risk patients.
• Collaborate with physicians, nurses, and case managers to coordinate plans.
• Document interventions per organizational standards.
• Support population-health and disease-management programs.
