Equity Staffing Group is seeking Clinical Pharmacists to join its Clinical Engagement Services team, supporting MTM, adherence programs, diabetes care, and polypharmacy management. This is a remote role with flexible schedules.

You will conduct telephonic reviews, identify care gaps, educate patients, and communicate recommendations to providers, while documenting interventions. Requirements include a PharmD and active state license; remote work experience is highly valued.

Qualifications

• Must hold current, unrestricted Pharmacist license in home state.
• Strong knowledge of MTM and disease-state management.
• At least 1 year of remote/independent work experience.
• Strong communication, organization, and technology skills.
• PharmD required; PBM/retail experience and bilingual skills are a plus.
• Additional active Pharmacist licenses in several states are highly preferred.

Responsabilités

• Conduct telephonic consultations and comprehensive medication reviews
• Identify gaps in care, adherence concerns, drug interactions, and high-risk medications
• Provide medication and disease-state education
• Communicate clinical recommendations to patients and healthcare providers
• Document interventions and recommendations accurately
