Job summary

AnewHealth is seeking a Clinical Pharmacist to perform comprehensive medication safety reviews for PACE participants. This remote role emphasizes optimizing regimens, identifying drug-related problems, and coordinating with providers and the interdisciplinary team to improve outcomes.

The position requires ACPE-accredited education, active pharmacy licensure, and a geriatrics interest or certification. Experience in pharmacogenomics, polypharmacy, and patient-centered care is preferred.

Qualifications

• Graduate of an ACPE-accredited pharmacy program or equivalent.
• Current, active pharmacist license in good standing.
• Board certification in geriatrics preferred.

Responsibilities

• Conduct comprehensive Medication Safety Reviews (MSRs) for PACE participants using proprietary platforms.
• Complete medication reviews and reconciliation for enrollments, changes, and high‑risk events.
• Identify and resolve medication-related problems including polypharmacy and adherence barriers.
• Collaborate with IDT and providers to implement optimization and deprescribing when appropriate.
• Monitor high-risk meds, ensure timely follow-up and documentation of outcomes.
• Participate in meetings, quality initiatives, and educational programs while ensuring regulatory compliance.
