Job summary

GalenusCare seeks a Clinical Optimization Pharmacist to join their PACE/Hospice interdisciplinary care team in Mt. Laurel, NJ. This full-time position involves providing comprehensive medication management and clinical support to patients and caregivers. The role requires a PharmD and at least three years of clinical practice in geriatrics or chronic care. Qualifications

• Minimum three years of experience as a clinical pharmacist in geriatrics or chronic care management.
• Prior experience in clinical support or call center setting is preferred.

Responsibilities

• Conduct continuous precision polypharmacy reviews.
• Provide evidence-based medication recommendations to providers.
• Monitor high-risk and high-cost medications.

Clinical knowledge of geriatric patient care and medication management

Excellent communication skills

Ability to multitask in a high-volume environment

Education

Doctor of Pharmacy (PharmD)

Board Certification in Geriatric Pharmacy (BCGP)
