Job summary

AnewHealth seeks a Pharmacist for a PRN, first-shift role to oversee pharmacotherapy outcomes and medication distribution. You will collaborate with care teams to address medication-related problems and ensure compliant medication handling through proprietary software.

Responsibilities include patient counseling, care plan explanations, and accurate documentation for billing and records. Prior experience in geriatrics or chronic care is preferred; licenses must be active and in good standing.

Qualifications

• Graduate from a pharmacy program accredited by ACPE or qualifies to practice
• Active license/registration in good standing with the state board
• Experience in pharmacy distribution, packaging, geriatrics or chronic care preferred

Responsibilities

• Serve as primary contact for patients, families, and healthcare providers to address medication-related problems
• Explain care plan options, uses, side effects and contraindications of medications
• Ensure proper documentation of care decisions, billing and patient records
• Oversee technicians in medication reconciliation, order entry, inventory, storage and labeling
• Perform timely review and preparation of prescriptions with accuracy and safety
