Job summary

AnewHealth is seeking a Pharmacist for CareKinesis Clinical Initiatives to optimize medications for PACE participants. You will conduct MSRs, reconcile medications, identify issues, and work with providers to improve outcomes. Remote, full-time role with a focus on geriatrics and high-risk populations.

Ideal candidates have geriatric pharmacotherapy expertise, a valid U.S. license, and proficiency with clinical platforms and pharmacy software. HIPAA and patient safety compliance are essential.

Qualifications

• ACPE-accredited PharmD or foreign equivalent with license to practice in the U.S.
• Experience in geriatrics and polypharmacy management preferred.
• Strong knowledge of HIPAA, FWA, and patient safety requirements.

Responsibilities

• Conduct comprehensive medication safety reviews (MSRs) for PACE participants.
• Complete medication reviews and reconciliations for enrollments and transitions.
• Identify and resolve medication-related problems including polypharmacy and ADEs.
• Collaborate with IDT and providers to optimize therapy and deprescribing when appropriate.
• Document outcomes and monitor progress toward therapeutic goals.
• Participate in meetings, QI initiatives, and comply with regulations.
