Job summary

Mercy Health in Cincinnati seeks an experienced RN for a primarily remote role with on-site visits as needed to support clinical integration and value-based care in a regional network.

You will guide providers and care teams, translate data into actionable insights, and drive Plan-Do-Study-Act improvements to enhance patient outcomes and efficiency. Occasional travel to meetings may be required.

Qualifications

• RN licensure in good standing with US or state board.
• BSN required; MSN preferred.
• Minimum 5 years in healthcare with leadership in Population Health or value-based work.
• Experience with process improvement models such as PDSA and A3.

Responsibilities

• Completes in person and virtual rounds to share status reports and insights.
• Reviews clinical outcomes, patient panel management, and performance metrics.
• Partners with providers to implement evidence-based protocols and improvements.
• Coaches regional staff on practice transformation and measurement tools.
• Facilitates onboarding and integration of providers into networks.
