Job summary

Mercy Health is seeking an experienced RN to support clinical integration efforts across Charleston and Florence, SC. The role emphasizes driving improvements through data-driven insights, coaching care teams, and coordinating with providers and affiliates in a value-based care model.

Responsibilities include facilitating PDSA cycles, guiding workflow changes, and monitoring outcomes across the market network, ensuring adherence to evidence-based protocols and performance metrics.

Qualifications

• Current RN licensure (CCNE/ACEN/CNEA preferred)
• BSN required; MSN/MBA or related preferred
• 5 years healthcare experience with management or population health

Responsibilities

• Hire must be local to Charleston area for onsite work.
• Conduct in-person and virtual rounds; communicate status, drivers, and insights to providers.
• Review clinical outcomes, patient panel management, and performance metrics for affiliates and physicians.
• Partner with providers to implement evidence-based protocols; monitor progress and resources.
• Act as role model for practice transformation and educate stakeholders on tools and tasks.
• Lead provider onboarding/offboarding within network and manage panel assignments.
• Support value-based care participation and workflows with regional leadership.
• Collaborate to implement network initiatives and improve clinical integration processes.
• Conduct chart audits to support quality reviews and Epic/non-Epic distributions.
