Job summary

Mercy Health in Ohio is seeking a Social Worker to deliver clinical care management, coordinating care to improve health outcomes and reduce unnecessary costs. You will work with the Interdisciplinary Team to identify patient needs and connect them with benefits and resources.

This remote role supports OH, KY, and IL; you will maintain caseloads, develop care plans, and collaborate with PCPs, specialists, and hospitalists to implement patient-centered care.

Qualifications

• Bachelor’s degree in Social Work required
• Master’s degree or licensure as required by state of practice
• Case Management certification preferred
• 2–3 years acute care, home health or case management experience
• Excellent interpersonal communication and negotiation skills
• Strong analytical, data management and computer skills
• Experience with database entry, EMR documentation, PowerPoint and basic Excel

Responsibilities

• Maintain a caseload of patients according to department policies
• Identify, enroll and manage patients in the Complex Case Management program
• Develop and implement care plans to maximize wellbeing with regular reviews
• Collaborate with ACM, PCPs, Specialists, and Hospitalists on patient-centered care
• Assess social determinants of care and set goals within the care plan
• Document communications in electronic medical records
• Coordinate services for disabled status or post-acute placement
• Advocate for patients and connect to community resources
