Job summary

Mercy Health seeks an experienced clinical integration leader to guide providers, care teams, and affiliates in a value-based care model. You will translate data into actionable insights, drive improvement via PDSA cycles, and coach teams across the Cincinnati market.

The role emphasizes remote work with regular onsite rounds and collaboration with regional leaders. Strong nursing leadership, process-improvement skills, and EPIC EMR proficiency are required.

Qualifications

• 5 years of healthcare experience in ambulatory settings with leadership in Population Health or value-based contracts
• Familiarity with process improvement models such as PDSA and A3
• Experience with value-based arrangements and clinical reporting packages (e.g., Milliman, Tableau)

Responsibilities

• Conduct in-person and virtual rounds to communicate status and insights to providers and practices
• Review clinical outcomes, patient panels, and performance metrics for affiliates and employed physicians
• Partner with care teams to implement evidence-based protocols and monitor progress
• Educate regional stakeholders on process improvements and tools for care transformation
• Onboard and offboard providers within the network related to panel management and initiatives
• Lead initiatives to drive value-based care participation and regional network implementation
• Coordinate chart audits and quality reviews to support Mercy Health Select and related programs
• Maintain relationships with network leadership to support ministry outcomes and care delivery
