Job summary

You will work primarily remote with occasional onsite rounds; local to Cincinnati, OH, and travel as needed. Strong collaboration with regional stakeholders is essential to success.

Qualifications

• 5 years of healthcare experience with management or leadership in Population Health or value-based care.
• Familiarity with PDSA/A3 process improvement methods preferred.
• Experience with clinical reporting packages (Tableau, Milliman) preferred.

Responsibilities

• Perform rounds and provide status updates, leading/lagging indicators, and insights to practices.
• Review clinical outcomes and performance metrics for affiliates and physicians.
• Collaborate with providers to implement evidence-based protocols and improvement initiatives.
