Cityblock Health in Cincinnati/Columbus offers a remote-friendly role with in-person visits to the hub 1–2 times per week. You will lead comprehensive intake visits and chronic disease management within an interdisciplinary team to deliver wrap-around care and preventive services.

You will engage in care coordination, documentation, and evidence-based protocols, while supporting member goals and participating in program development and policy improvements.

Formación

• Accredited Nurse Practitioner or Physician Assistant program with national certification.
• Current valid medical license to practice in the state of employment.
• National certification as a Physician Assistant (NCCPA) or equivalent.

Responsabilidades

• Conduct comprehensive intake visits and provide direct primary care with focus on chronic disease management and preventive care.
• Outreach to members for follow-up care and accurate medical coding (HCC, ICD-10, CPT).
• Prescribe medications for depression, anxiety, and substance use disorders as appropriate.
• Stay updated on evidence-based care for complex patients and refer to specialists as needed.
• Participate in case conferences to support care plan development and health maintenance.
