Job summary

Cadence Health is hiring a Clinical Navigator (Medical Assistant) for remote, US-based work to support Medicare patients in Cadence programs.

You will perform proactive outreach, coaching, and care coordination to close gaps in care, document in the Cadence platform, and collaborate with clinicians to ensure safe, effective care. Strong MA background and remote work readiness are required.

Qualifications

• Active Medical Assistant certification from an accredited association.
• Minimum 5 years as a Medical Assistant.
• Remote work readiness with stable high-speed WiFi.
• Experience in internal medicine/cardiology/geriatric care preferred.
• Spanish fluency strongly preferred.
• Experience using AI-assisted clinical tools and discussing AI in care delivery.

Responsibilities

• Conduct outbound telephonic outreach and one-on-one coaching for chronic condition patients.
• Educate and coach on behavior change, nutrition, activity, and self-management.
• Assess health status and readiness to change.
• Educate on disease management and medication adherence; follow Cadence protocols.
• Monitor progress and outcomes via remote tools and Cadence platform.
• Document interactions and coordinate with clinical teams for next steps.
• Collaborate in real time with clinicians using Cadence Platform, Google Workspace, and Slack.
