Cadence is seeking a Bilingual/Spanish Speaking Clinical Navigator (Medical Assistant) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will document patient interactions in the Cadence platform and collaborate with the clinical team to coordinate next steps.

Qualifications

• Spanish-speaking required.
• Active Medical Assistant certification from an accredited association is required.
• At least 5 years of experience as a Medical Assistant is required.
• Remote work readiness with stable high-speed WiFi is required.
• Experience in internal medicine, cardiology, nephrology or geriatrics preferred.
• Experience educating patients with chronic conditions preferred.

Responsabilités

• Conduct outbound telephonic patient outreach and provide coaching to patients with chronic conditions.
• Educate and coach patients on behavior change, nutrition, activity, and self-management using evidence-based techniques.
• Assess patients' health status, lifestyle, and readiness to change.
• Educate on disease management, medication adherence, and prevention; follow Cadence protocols under supervision.
• Monitor progress and health outcomes via regular check-ins and remote tools.
• Document patient interactions and follow-up plans; connect patients with resources as needed.
• Collaborate with clinicians using Cadence Platform, Google Workspace, and Slack.
