Job summary

Cadence is a clinical AI company delivering virtual chronic care support for Medicare patients enrolled in Cadence programs. We are hiring a Clinical Navigator (Licensed Practical Nurse) to provide remote, one-on-one coaching and care coordination for chronic conditions.

The role is full-time and remote within the United States, with set hours across Central, Mountain, and Pacific time zones. Compensation ranges hourly, with potential incentive rewards; benefits include comprehensive health

Qualifications

• Active compact multi-state LPN/LVN license is required.
• Minimum of 5 years of clinical experience as an LPN/LVN is required.
• Remote work readiness with stable high‑speed WiFi is required.
• CA state LVN license is highly preferred.
• Fluency in Spanish is strongly preferred.
• Experience providing patient education and support for chronic conditions is preferred.
• Fluency with AI-assisted clinical tools (Gemini, Open Evidence or similar) and ability to discuss AI in care delivery.

Responsibilities

• Outbound telephonic patient outreach and one-on-one coaching for chronic conditions.
• Educate and coach patients on behavior modification, nutrition, activity, and self-management using evidence-based techniques.
• Assess health status, lifestyle, nutrition, and readiness to change.
• Educate on disease management, medication adherence, symptoms, and prevention; follow Cadence policies under supervision.
• Monitor progress and health outcomes via check-ins and remote monitoring tools.
• Document interactions, outcomes, and follow-ups; connect patients with resources as needed.
• Collaborate in real time with clinicians and support teams using Cadence Platform, Google Workspace and Slack.
