Cadence Health is seeking a bilingual Clinical Navigator (Medical Assistant) to deliver remote, Medicare-focused chronic care support. You will conduct proactive patient outreach, coach patients on behavior modification, and coordinate care with clinicians, all while documenting in the Cadence platform.

This role requires a Medical Assistant certification, 5+ years of MA experience, and Spanish fluency. This full-time, remote position offers standard M-F hours with US-based time zones.

Qualifications

• Active Medical Assistant certification from an accredited association is required.
• At least 5 years of experience as a Medical Assistant is required.
• Remote work readiness (including stable high-speed WiFi) is required.
• Experience in internal medicine, cardiology, nephrology or geriatric care is preferred.
• Fluency in Spanish is required.
• Experience with AI-assisted clinical tools (e.g., Gemini, Open Evidence) is preferred.

Responsabilités

• Conduct outbound telephonic outreach and provide one-on-one coaching to chronic disease patients.
• Educate patients on disease management, self-management, and goal-setting using evidence-based approaches.
• Assess health status and readiness to change; document interactions in Cadence platform.
• Coordinate care, monitor progress, and address barriers to care in collaboration with clinicians.
