Job summary

Cadence is seeking a Clinical Navigator (Medical Assistant) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct patient outreach, coaching, and care coordination to close gaps in care, while using Cadence platform, Google Workspace, and Slack to collaborate with the clinical team.

The role is full-time and remote, based in the United States, with standard hours 8:00 AM–5:00 PM PT or 9:00 AM–6:00 PM MT/PT.

Qualifications

• Active Medical Assistant certification from an accredited association (AAMA, AMCA, AMT, NHA, NAHP, NCCT, NRCMA/NRCAMA or AAH).
• At least 5 years of experience as a Medical Assistant.
• Remote work readiness with stable high-speed WiFi required.
• Fluency in Spanish strongly preferred.
• Experience with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to discuss AI-supported care vs. human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
• Coach patients on behavior change, nutrition, activity, and self-management using evidence-based techniques like motivational interviewing.
• Perform comprehensive assessments of health status and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention per Cadence policies.
• Monitor progress and outcomes via check-ins and remote tools.
