Job summary

Cadence is recruiting a Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This remote role emphasizes proactive coaching, care coordination, and documentation within the Cadence platform, with collaboration across the clinical team to ensure patient engagement.

You will conduct outreach, assess health status, coach on behavior change, and support medication adherence while leveraging AI-assisted tools and standard

Qualifications

• Active multi-state LPN/LVN license required.
• Minimum 5 years of clinical LPN experience.
• Remote work readiness with stable high-speed internet.
• Spanish fluency strongly preferred.
• Experience educating patients with chronic conditions preferred.
• Fluency with AI-assisted clinical tools and ability to discuss AI support with providers and patients.

Responsibilities

• Conduct outbound outreach and coaching for patients with chronic conditions.
• Educate and coach patients on personalized care plans and behavior change.
• Assess health status, lifestyle, and readiness to change.
• Educate on disease management, medication adherence, and prevention.
• Monitor progress and document outcomes in Cadence platform.
• Collaborate with clinicians using Cadence, Google Workspace, and Slack; provide feedback to improve workflows.
