Job summary

Cadence is hiring a bilingual Clinical Navigator (Medical Assistant) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will conduct proactive outreach, coaching, and care coordination, documenting interactions in the Cadence platform and collaborating with the clinical team.

The role requires a Medical Assistant certification, 5+ years of experience, and Spanish fluency.

Qualifications

• Active Medical Assistant certification from an accredited association is required.
• At least 5 years of experience as a Medical Assistant is required.
• Remote work readiness (stable high‑speed WiFi) is required.
• Experience in internal medicine, cardiology, nephrology or geriatric care is preferred.
• Experience providing patient education and support to individuals with chronic conditions is preferred.
• Fluency in Spanish is required.
• Fluency with AI-assisted clinical tools (Gemini, Open Evidence) and ability to discuss AI-supported care vs. human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching to patients managing chronic conditions.
• Provide education and coaching focused on behavior modification, nutrition, physical activity, and self-management using motivational interviewing and SMART goals.
• Conduct comprehensive assessments of health status, behaviors, and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention; follow Cadence protocols.
• Monitor progress and health outcomes through regular check-ins and remote tools.
• Document patient interactions and follow-up plans; identify barriers and connect to resources.
• Collaborate in real time with clinicians and support teams using Cadence Platform, Google Workspace, and Slack.
