Qualifications

• Active nursing license in current state
• Minimum two years clinical experience in clinic, Med/Surg, case management or home health
• Experience with Electronic Medical Records and HIPAA compliant remote work

Responsibilities

• Manage a patient panel with focus on care coordination and billable encounters
• Perform 20-minute unit-based care encounters and document thoroughly
• Meet productivity targets and participate in ongoing clinical updates
