Job summary

Harris Computer is seeking a Remote Care Manager to oversee a patient panel, operating from home. You will conduct care coordination, motivational coaching, and documentation to drive chronic care management, with a productivity-based pay structure tied to patient encounters.

The role requires an active nursing license, 2+ years of clinical experience, and strong EMR and Office 365 skills. Remote work offers autonomy and flexible scheduling.

Qualifications

• Active RN/LPN/LVN compact or full license with no disciplinary actions.
• Minimum two years in clinics, Med/Surg, case management or home health.
• Experience with EMR and HIPAA compliant remote care workflows.
• Strong communication, organization and time management.
• Ability to learn new technology via tutorials in a self‑directed way.

Responsibilities

• Abide by plan of care and orders from the practice.
• Provide prevention and intervention for chronic conditions via motivational coaching.
• Develop positive interactions with patients and support care teams.
• Improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients and attend mandatory meetings.
