Qualifications

• RN/LPN/LVN/BSN with active license in home state or compact state.
• 2+ years clinical experience in clinic, Med/Surg, Case Management, or home health care.
• Experience with electronic medical records and HIPAA compliant workflows.

Responsibilities

• Maintain a patient panel of 30–50 patients per month in the first three months.
• Complete due diligence and billable encounters for assigned patients.
• Document care plans and care coordination activities in EMR.
