Job summary

Harris Computer is seeking a Remote Care Manager to oversee patient panels and drive billable care management encounters. The role emphasizes productivity-based pay with escalating incentives and requires RN/LPN/LVN with a compact license, EMR proficiency, and strong communication skills.

Expect a fully remote setup with flexible scheduling, a focus on chronic disease management, and participation in regular meetings.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.)
• Current COMPACT license to practice as an RN/LVN/LPN with state residency
• Minimum of two years of clinical experience in a clinic setting, Med/Surg, Case Management, or home health care
• Experience with Electronic Medical Records and Microsoft Office 365; HIPAA-compliant home environment
• Strong time-management, initiative, organization and problem-solving skills
• High-speed internet (18 Mbps down / 6 Mbps up) and Windows or Mac computer; no tablets

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic disease via motivational coaching.
• Develop positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Care Management episodes for chronic disease management.
• Develop detailed care plans for doctors and patients for prevention and intervention.
• Understand health care goals for chronic disease management.
• Attend regularly scheduled meetings to define scope of work.
