Job summary

Harris Computer is seeking a Remote Care Manager to support a patient panel and provide coaching for chronic disease management across assigned practices.

The role is remote with a productivity-based pay structure based on patient encounters. It requires RN/LPN/LVN licensure, EMR proficiency, HIPAA compliance, and strong communication. A fast learner with reliable internet and a Windows/Mac setup is essential.

Qualifications

• RN/LPN/LVn with active COMPACT license in current state
• Minimum 2 years clinical experience in clinic/Medsurg/Case Management/home health
• Hands-on experience with Electronic Medical Records and HIPAA-compliant remote work
• Excellent initiative, organization, time-management and problem-solving
• Able to quickly learn new technology via tutorials in a self-led environment
• High-speed internet and Windows/Mac desktop or laptop (no Chromebooks)
• Excellent verbal and written communication skills

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic disease management through motivational coaching.
• Develop positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients for prevention and intervention.
• Understand health care goals for chronic disease management.
• Attend mandatory meetings (Monthly Clinical Update, 1:1 with supervisor).
