Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.).
• Current COMPACT license to practice as an RN/LVN/LPN in the resident state.
• At least two years of clinical experience in clinic, Med/Surg, case management or home health care.

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic disease through coaching.
• Develop positive interactions with patients and improve revenue through billable Care Management episodes.
• Create detailed care plans for doctors and patients and coordinate care accordingly.
