Job summary

Esrun Health is seeking a Remote Care Manager to oversee a patient panel, coordinate care, and drive chronic disease management outcomes. You will perform chart reviews, collect data, and ensure 100% due diligence on assigned patients while maintaining HIPAA compliance.

The role requires nursing licensure in compact states, at least two years of clinic or home health experience, and strong EMR skills. You'll work remotely with flexible hours, participate in regular meetings, and focus on

Qualifications

• RN/LPN/LVN with COMPACT license in current state
• Minimum 2 years clinical experience in clinic/home health
• Proficient with EMR and MS Office
• Able to work remotely with HIPAA compliance

Responsibilities

• Abide by plan of care and orders of the practice
• Provide prevention and intervention for chronic conditions
• Develop detailed care plans for doctors and patients
• Attend mandatory meetings (Monthly Clinical Update, 1:1 with supervisor)
• Develop billable Care Management episodes to improve revenue
