Job summary

Aylo Health Services, Inc. is seeking a compassionate Care Advisor to support our Chronic Care Management program in a remote capacity. You will coordinate care for patients with chronic conditions, conduct regular check-ins, and develop care plans ensuring high-quality metrics.

Qualifications

• Medical Assistant certification strongly preferred.
• Minimum 3 years of clinical experience in a healthcare setting.
• Strong knowledge of chronic conditions and care management.
• Proficiency in EHR systems and familiarity with clinical workflows.
• Excellent communication and interpersonal skills.
• Experience in patient education and care coordination.
• Ability to operate independently.

Responsibilities

• Conduct regular check-ins with the assigned patient panel via telephone.
• Develop and maintain comprehensive care plans.
• Coordinate care between healthcare providers and services.
• Document all patient interactions in EHR system.
• Monitor and track patient progress and compliance.
• Identify and address barriers to care.
• Provide patient education on chronic condition management.
