Aylo Health is seeking a compassionate Care Advisor to join our Chronic Care Management (CCM) team. The ideal candidate will provide comprehensive care coordination and support to patients with chronic conditions, ensuring they receive optimal care while meeting quality metrics and program goals.

The role is full-time and remote with a preference for candidates located in Georgia or neighboring states. The position requires clinical experience, strong EHR knowledge, and excellent communication

Qualifications

• 3+ years of clinical experience in a healthcare setting.
• Strong understanding of chronic conditions and care management.
• Proficient with EHR systems and clinical workflows.
• Excellent communication and interpersonal skills.
• Experience educating patients and coordinating care.
• Ability to work independently and manage multiple patients.

Responsibilities

• Conduct regular check-ins with the assigned patient panel via telephone.
• Develop and maintain comprehensive care plans.
• Coordinate care between healthcare providers and services.
• Document all patient interactions in the EHR system.
• Monitor patient progress and adherence to plans.
• Identify and address barriers to care.
• Provide patient education on chronic condition management.
• Meet monthly performance metrics and quality goals.
