Job summary

VytlOne is seeking a dedicated Case Manager with pharmacy technician experience to support patient care in a growing Specialty Pharmacy. You will manage demographics, co-pay support, and delivery coordination while collaborating with pharmacists and the sales team.

Responsibilities include patient intake assessments, clinical documentation, and proactive outreach for new prescriptions. A focus on accuracy, communication, and problem solving is essential in a dynamic work-from-home environment.

Qualifications

• High School diploma or equivalency.
• Registered/Certified Pharmacy Technician as required by SBOP.
• One to two years of relevant experience in a pharmacy/retail setting.
• Ability to use pharmacy software and maintain patient data accurately.
• Strong communication and organizational abilities.

Responsibilities

• Obtain copy of prescription drug card and file in electronic chart.
• Ensure patient demographic information is accurate in the pharmacy system.
• Set up patient copay assistance and foundation support when necessary.
• Manage daily work orders and communicate any issues to patients, prescribers, and sales teams.
• Contact patients to schedule initial shipments and educate on device maintenance when applicable.
• Conduct Case Management Assessment for new referrals and create initial/ongoing clinical assessments.
• Prepare delivery tickets and ensure correct shipment addresses and supplies.
• Collect payment information and log it for future use in CPR+.
• Prepare shipment status updates for prescribers as needed.
• Handle incoming calls and manage assigned queues in SPRX.
• Support claims adjudication and benefits verification for new Rx.
