Qualifications

• Residency training or 2 years post-graduate pharmacist experience in specialty pharmacy, ambulatory care, managed care, infusion, retail, hospital, or other clinically relevant setting.
• BS or Pharm. D. from an accredited school of pharmacy; current, unrestricted license in the primary state of practice; ability to obtain additional licenses.
• Specialty pharmacy, immunization, medication therapy management, sterile compounding or related certifications may be required or preferred.

Responsabilités

• Interpret and fill drug orders accurately and timely.
• Ensure appropriateness of all medication orders with patient factors and lab data.
• Conduct Drug Utilization Reviews for safety and interactions.
• Review prior authorization requests using predefined criteria.
• Provide patient counseling and drug information to staff and patients.
• Collaborate with physicians and internal teams on clinical initiatives.
• Supervise pharmacy technicians and oversee daily operations.
• Generate and review monthly/quarterly reports; participate in quality improvement.
