Qualifications

• Minimum 2 years in medical social work or case management.
• Must travel up to 25–50% of the time in Wayne and Macomb Counties.
• Must comply with state requirements where duties are performed.
• Basic problem solving and decision making skills.
• Basic digital literacy.
• Basic knowledge of medical terminology.
• Ability to deal tactfully with customers and community.
• Ability to handle sensitive information ethically and responsibly.

Responsabilités

• Coordinates care for members with acute or chronic conditions, focusing on quality of care, outcomes and costs.
• Performs initial and periodic assessments for Long-Term Care/CM programs.
• Develops individualized case plans for at-risk members.
• Communicates updates with providers, PCPs, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive necessary care under their benefit plan.
• Provides information and education on social issues, mental health, and available services.
• Develops complex programs to reduce admissions and improve outcomes.
• Coaches junior colleagues in social work techniques.
• Collaborates with community members and policymakers to address issues.
