CVS Health seeks a Care Coordinator to manage long-term rehabilitation and psychosocial needs, coordinating care, resources, and services to prevent readmissions. You will work remotely with significant travel (25-50%) across Wayne and Macomb Counties, ensuring quality outcomes for members.

The role requires a social work background, at least 2 years in case management, and ability to develop individualized plans while collaborating with multidisciplinary teams.

Qualificações

• Minimum 2 years experience in medical social work or case management.
• Complies with all state requirements in the state where job duties are performed.
• Must be willing and able to travel up to 25%-50% of the time in Wayne and Macomb Counties.
• Basic awareness of problem solving and decision making skills.
• Basic awareness of digital literacy skills.
• Basic knowledge of medical terminology.
• Ability to deal tactfully with customers and community.
• Ability to handle sensitive information ethically and responsibly.

Responsabilidades

• Controls comprehensive management of members with acute or chronic conditions, focusing on quality of care and outcomes.
• Implements initial and periodic assessments for Long-Term Care Program/case management.
• Develops individualized case plans for at-risk members.
• Communicates updates with providers, physicians, and care management staff.
• Identifies opportunities to ensure members receive needed care under their plan.
• Provides information and education on social issues and available services.
• Develops programs to reduce admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and organizations to address issues.
