Job summary

Premera Blue Cross is hiring Case Managers for a fully remote, telecommuter role in the United States. You will support members through a whole-person care model, assess needs, coordinate care, and help remove barriers to healthier daily living.

You will join a collaborative team, receive extensive training, and have access to a broad total rewards package including education support and CCM resources. The position offers flexible scheduling with dedicated training periods.

Qualifications

• Active WA State or Compact RN or ARNP license that allows independent assessment.
• 3 years’ post licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination or discharge planning.

Responsibilities

• Engage members who are facing medical utilization disorders and build a relationship that supports open dialogue.
• Conduct holistic health assessments to identify SDOH and cultural influences that affect lifestyle changes.
• Create a member-centric case management plan with goals and address potential barriers.
• Monitor and update the case management plan to ensure progress toward goals.
• Facilitate communication among the health care team to maximize appropriate care delivery.
• Provide interventions to remove barriers and promote positive behavior change using motivational interviewing.
