BlueCross BlueShield of Tennessee is seeking a dedicated Case Manager to join our team. This remote RN will manage complex members telephonically and digitally, addressing significant medical and psychosocial needs to ensure timely and effective interventions.

You will apply clinical judgment, coordinate with multidisciplinary resources, and provide follow-up to optimize outcomes. Strong communication, digital proficiency, and willingness to leverage AI tools to improve care are essential.

Qualifications

• Registered Nurse (RN) licensed in TN or eligible per state compact.
• Excellent written and verbal communication skills.
• Strong digital literacy and comfort with AI-based care tools.

Responsabilités

• Support utilization management for complex and non-routine cases.
• Act as liaison between members, providers and internal/external customers.
• Oversee highly complex cases to ensure effective interventions and efficient benefit use.
• Perform core case management activities: assessment, planning, implementation, coordination, monitoring, outcomes and evaluation.
• Communicate effectively via digital channels and provide technical support.
