CVS Health is seeking a remote RN Case Manager to support members nationwide via telephonic care coordination. This full-time role requires applying clinical guidelines, coordinating benefits, and collaborating with the care team to improve outcomes.

A minimum of 5 years' RN experience and an active multistate license is required. The position offers comprehensive benefits and remote work across the United States, with 40 hours per week and virtual training to start.

Qualifications

• 5+ years' experience as a Registered Nurse.
• Active multistate license via the Nurse Licensure Compact (NLC).
• 1+ years' experience documenting electronically.
• 1+ years' experience in Oncology.

Responsibilities

• Telephonic case management with patients and care teams.
• Apply clinical guidelines, care plans, policies and regulatory standards while assessing benefits.
• Address barriers and risks to care planning and member outcomes.
• Assess conditions including co-morbid illnesses impacting functionality.
• Consult with supervisors and multidisciplinary teams to meet goals.
• Use motivational interviewing to engage members and assess needs.
