Job summary

Premera Blue Cross seeks a Telecommuter Case Manager to support members through a whole‑person care model. You will assess needs, collaborate with caregivers and providers, and develop a member‑centric plan aimed at improved health outcomes.

You will participate in extensive training, pursue CCM/ICM credentials if not yet held, and contribute to a program recognized for NCQA accreditation and LEAN process improvements.

Qualifications

• Active WA State or Compact RN or ARNP license that allows you to conduct an assessment independently.
• 3 years' post licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time.
• CCM certification preferred and must be obtained within 24 months of hire.
• Bachelor’s or Master’s degree required.
• Integrated Case Management certificate (ICM) required.
• Chronic Condition Certifications (Asthma, Cardiology, Diabetes, High Risk Maternity, Oncology, Pediatrics) preferred.

Responsibilities

• Engage members facing medical utilization disorders and build a relationship for open dialogue on sensitive topics.
• Conduct initial holistic health assessments to understand needs and social determinants of health.
• Create a member-centric case management plan with caregivers and providers to set goals and address barriers.
• Monitor and update the case management plan to ensure progress toward goals.
• Facilitate communication among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote behavior change using motivational interviewing.
• Maintain concise, accurate documentation meeting accreditation and contractual requirements.
