Premera Blue Cross is seeking a licensed registered nurse (WA state or compact) to join our team as a Case Manager. This telecommuter role supports members through holistic, whole-person care, addressing social determinants of health and coordinating care with providers.

You will conduct initial assessments, help develop member-centric care plans, monitor progress, and maintain timely documentation while engaging with care teams and caregivers.

Qualifikationen

• Active WA State or Compact RN license that allows independent assessments.
• 3 years' post-licensure direct care experience.

• Engage and assess members to understand medical utilization and social determinants of health.
• Create member-centric case management plans with caregivers and providers.
• Monitor progress and update care plans; document accurately and timely.
• Coordinate between health care team to ensure appropriate care delivery.
