Job summary

CVS Health Corporation is seeking a Case Manager RN for a 100% remote role, open to candidates in any state. You will assess, plan, coordinate, and advocate for members’ benefit plans and health needs, delivering high-quality, cost-effective care with strong collaboration and communication.

The role requires an active RN license, 3+ years of acute care experience, and flexibility for occasional shift adjustments. Work from home setup must meet CVS Health standards; no travel is anticipated.

Qualifications

• Active RN license in state of residence.
• Willingness to obtain additional state licenses upon hire (company paid).
• 3+ years of acute care experience as an RN.
• Must be available to work Monday–Friday 8:00am–4:30pm in residence time zone with occasional late shift rotation.

Responsibilities

• Apply data-driven methods to identify members for individualized case management.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on member needs and plans.
• Advocate for patients to maximize health coverage.
• Promote cost-effective outcomes and program efficiency.
• Identify quality issues and escalate as needed.
• Provide high level customer service.
• Assess health literacy and readiness to change.
• Provide coaching and support for independent medical and healthy lifestyle choices.
