Job summary

CVS Health is seeking an experienced RN Case Manager for a 100% remote role. You will assess, plan, and coordinate care for members telephonically or virtually, developing personalized plans and connecting with benefits and community resources.

Requirements include an active RN license, 3+ years of acute care RN experience, and willingness to obtain additional state licenses. Remote work nationwide with a strong emphasis on patient engagement and case management.

Qualifications

• Active, unrestricted RN license in your state of residence.
• Willingness to obtain additional state licenses (company-paid).
• 3+ years of acute care RN experience (medical, surgical, specialty, ICU, pediatrics, discharge planning, or case management)
• Comfortable working with multiple computer applications and monitors.
• Associate Degree in nursing.

Responsibilities

• Assess, plan, implement, and coordinate care for members telephonically and/or virtually.
• Develop individualized care plans and wellness strategies.
• Identify clinical and social barriers to health outcomes.
• Connect members with available programs, benefits, and community resources.
• Apply clinical judgment to manage complex health conditions and reduce risk factors.
• Engage members through strong communication and motivational interviewing techniques.
