Job summary

Humana is seeking a Telephonic Case Manager to work from home in Wisconsin. You will assess members’ medical, behavioral, and social needs, develop and monitor care plans, and coordinate with physicians and providers.

You will educate members on benefits and services, document interactions, and support iCare team activities. Responsibilities include post-hospital follow-ups, facilitating care coordination, and promoting preventive health guidelines.

Qualifications

• Bachelor's degree in health and human services field.
• 2+ years of case management experience.
• Must reside within the State of WI.

Responsibilities

• Conduct initial and annual assessments of members' medical, behavioral and social needs.
• Develop care plans addressing needs and coordinate with members and providers.
• Foster relationships with physicians and social service providers to support care coordination.
• Review and update care plans as indicated and monitor progress.
• Provide information on benefits, services and access protocols to members and providers.
