Humana, Inc. is seeking a Telephonic Case Manager working remotely from home in Wisconsin. You will assess members’ medical, behavioral, and social needs, develop care plans, and coordinate with providers to ensure proper interventions.

You’ll engage with a caring team under the Manager of Care Management Support. Requirements include a bachelor’s in health and human services and 2+ years of case management experience, with a preference for bilingual candidates and Medicare/Medicaid experience.

Qualificações

• Bachelor's degree in health and human services field.
• 2+ years of case management experience.
• Must reside within the State of WI.

Responsabilidades

• Conduct initial and annual assessments of members' medical, behavioral and social needs.
• Develop a care plan addressing needs and coordinate with members and providers.
• Builds relationships with physicians and social service providers to support care coordination.
• Review, monitor and update care plans as indicated.
• Educate members on benefits, services and access protocols.
