Formación

• Bachelor's degree in health and human services field.
• 2+ years of case management experience.
• Must reside within the State of WI.

Responsabilidades

• Conduct initial and annual assessments of members' medical, behavioral and social needs.
• Develop care plans and coordinate interventions with members and providers.
• Educate members on benefits, services, and appropriate use of medical services.
• Provide referrals to community resources and document interactions.
• Follow up after hospitalization or ER visits.
• Participate in interdisciplinary iCare team meetings.
